Assumption of Risk & Release of Liability
Revelstoke Fabrication Lab Society (“RFL”)
Membership Sign-Up for the Revelstoke Idea Factory Program (the “Program”)
• I acknowledge, agree, and represent that I am in good health, and in proper physical and mental condition to participate in the Program, and that if at any time I believe conditions to be unsafe, I will immediately discontinue further participation in the Program.
• I acknowledge and agree that my participation in the Program entails risks, including risks arising from human error or negligence, equipment or equipment malfunction, or exposure to harmful substances. The risks include physical or emotional injury, serious bodily harm, paralysis, death or damage to myself, third parties or property.
• I willingly assume all such risks and any expenses associated with such risks, including any medical expenses required as a result.
• I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless RFL, its affiliates, licensees, transferees, and each of their respective directors, officers, employees, volunteers and agents from any and all claims, actions, suits, demands, causes of action, losses, damages, injuries, expenses or liabilities which are in any way connected with my participation in the Program or RFL’s provision of the Program or other use of RFL’s equipment or facilities, including any claims which allege negligent acts or omissions by RFL. Should RFL or anyone acting on its behalf be required to incur legal fees or costs to enforce this agreement, I agree to indemnify and hold RFL harmless for all such fees and costs.

I have reviewed all of the policies and procedures listed on the RFL website with respect to participating in this Program and I agree to follow and abide by all such policies and procedures, health and safety regulations and instructions.
